Your Form refers to your win-loss record in the current match. Each win increases your Form by 1; each loss decreases it by 1. For example, if you win your first game, your Form becomes +1. If you then lose two consecutive games, your Form drops to -1. Your Form is reset to zero each time you enter the finals or playoffs.
